Birth Certificate & Social Security Documents for Your Baby.

Your midwife is required by VA law to file a birth certificate form for your baby.
The law allows <30 days, after the birth, to complete & mail the required form.
Your midwife will complete the Birth Certificate Form and review it with you
at your two (2) week postpartum visit. If you do not have a full name for your
baby at that time, your midwife will be in regular contact over the remaining
two weeks to finalize the form, review it again, then both you and she sign it.
How to get a Legal, Certified Copies of Birth Certificate
The VA Office of Vital Records does not automatically mail your baby's
Birth Certificate to you. In order to receive legal, Certified copies of
your baby's Birth Certificate, you must order them.

Order By Mail
- Turn around time is estimated at 1 to 2 weeks (5 to 10 business days) for
CERTIFIED VITAL RECORDS from the day the request is received IN THIS
OFFICE. However, people are urged to allow sufficient time for delivery
of all CERTIFIED VITAL RECORDS.

Walk-in
- Same day service is available to walk-in customers. This is the
fastest way to obtain CERTIFIED VITAL RECORDS. Upon arrival, you
will complete an application, pay the $12 required fee, and show the
processing clerk a valid,

Express
Delivery through the VitalChek Network - Obtain CERTIFIED VITAL RECORDS within 2 to 5 BUSINESS DAYS through this state's express
service. A service charge of
$49.25 must be paid by credit card, delivered by
UPS carrier service ONLY.
Get Your Baby's Social Security Card with a signature & a

The Birth Certificate Form includes a check box & signature line designating your permission for the VA Office of Vital Records to send birth and contact information about your baby to the U.S.
Social Security Administration. Your signature is required in order
for the SSA to process a Social Security Number (SSN) for your baby
and to then generate a Social Security Card..In 4-6 weeks, the baby's Social Security card is mailed directly to the mailing address indicated on the Birth Certificate Form.
If you move within that time frame, be certain to
report change of address with U.S. Postal Service
to ensure proper delivery of your baby's SS card.
